In case of difficult cholecystectomy where Calot’s triangle is having inflammation, fibrosis and dense adhesions causing inability to identify its structures, one should avoid dissecting shield of McElmoyle and do subtotal cholecystectomy instead total cholecystectomy to avoid biliary and vascular injuries.
